Choices are currently recruiting for Support Workers for our Supported Housing & extra care schemes in Telford. We accept applicants without health and social care qualifications, as we offer full training to all staff. If you can put the service user first, enriching lives through the delivery of person centred care and support services, promoting independence and achieving valued outcomes, then we would like you to join us.
Key Duties and Responsibilities:
The post holder will support adults with learning disabilities & older people living in their own apartments to achieve valued outcomes identified in their individual packages of care. The overall emphasis of the post will be to ensure that service users enjoy a high quality of life which promotes independence and enhances life choices.
– To be aware of each service user’s support plan and associated care plans and to deliver support and care in accordance with these plans. Eg. Escorting to community facilities, help with tenancy issues, assistance with medication, shopping and cleaning, personal care etc.
-To assist service users in achieving personal development and greater independence, by encouraging their involvement in all decisions affecting all aspects of their lives.
-At all times to afford service users the dignity and respect to which they are entitled and in particular, to recognise any special requirements associated with an individual’s race, culture or religion.
-To work with families and other agencies to ensure continuity of service and communications.
